Contact lens is a risk factor for microbial keratitis

Orthokeratology is an effective method of slowing down the progression of myopia and consequently preventing its formidable complications: rhegmatogenous retinal detachment and myopic maculopathy.

However, wearing any contact lenses has its risk factors for the development of other complications, the only serious one being microbial keratitis.

Non-compliance is the real risk factor

The real risk factor for microbial keratitis is not so much the wearing of contact lenses per se, but rather non-compliance with the specialist’s recommendations during the wearing process. Wherever a patient is entrusted with a part of the treatment process, compliance is a problem.

The traditional approach to this problem views compliance as a rather passive process that can only be influenced by recommendations.

However, this is not the case: it can be controlled and compliance control is a powerful tool for preventing complications. It is not just a recommendation but a system for creating and maintaining compliance.

ESACare® patient management system is designed to minimize complications during lens wear and consists of three main elements:

Patient skills monitoring

It is performed at each follow-up visitand consists of two parts:
  1. Practical (testing the patient’s lens handling skills).
  2. Theoretical (questions about what cannot be tested in the patient room).

Active follow-up control

This is to monitor follow-up visits, which should be at least two per year (we recommend three visits, every four months). It is clear that if the patient does not come, their skills cannot be checked.

Clinical vigilance for microbial keratitis

Microbial keratitis is rare and this therefore dulls vigilance. Relaxation is not an option! If certain complaints and biomicroscopic symptoms are present, the patient should be referred immediately to an ophthalmologist (in some situations, to the emergency room).
With this approach to patient management, the probability of complications, including microbial keratitis, is significantly reduced, as confirmed by our practical experience
